Position Summary:
In line with Beaumont Hospital’s strategic objective of enhancing patient flow, reducing length of stay, strengthening community integration, and improving rehabilitation outcomes, that the Assistant Director of Nursing (ADON) will have responsibility for Community Outreach and Rehabilitation Pathway Development. This role will provide senior nursing leadership across community-based services, facilitate integrated care pathways, and establish a dedicated rehabilitation service for suitable Beaumont Hospital patients.
Responsibilities:
Principle Duties and Responsibilities:
1.1 Management of Beaumont Care in the Home Service
The ADON in consultation with the Director of Nursing, will assume operational and strategic responsibility for the Beaumont Care in the Home (BIH) Service, ensuring:
- Effective governance and oversight of the service and its staff
- Safe and timely discharge of suitable patients from the acute setting.
- Development of community-based alternatives to hospital admission where appropriate.
- Monitoring of service performance, patient outcomes, and quality indicators.
1.2 Community Liaison and Integrated Care Development
The ADON in consultation with the Director of Nursing will act as the primary liaison between Beaumont Hospital and community healthcare services within the hospital catchment area.
Key responsibilities will include:
- Establishing and strengthening relationships with community stakeholders.
- Working collaboratively with Integrated Care Programme for Older Persons (ICPOP) teams.
- Developing links with Pathfinder services for patients attending Beaumont campus and facilitating discharge.
- Collaborating with Assistant Directors of Nursing for Public Health Nursing (PHN) services as appropriate.
- Identifying opportunities to expand community support services and improve care pathways.
- Facilitating integrated discharge planning and reducing delays in patient transitions from acute care.
1.3 Development of a Dedicated Rehabilitation Pathway
The ADON in consultation with the Director of Nursing will lead the implementation and oversight of a structured rehabilitation pathway for Beaumont Hospital patients. In collaboration with the Director of Nursing, the ADON will be required to design and build a patient rehabilitation model encompassing;
- Design and implement Rehabilitation Referral process
Operational Management
- The post holder will provide nursing leadership and professional advice that supports effective delivery of care across the hospital site.
- To coordinate efficient and effective resource utilisation ensuring value for money objectives are realised.
- To be accountable for the quality of service delivery, ensuring effective patient care within the service.
- To promote the development and maintenance of high standards of care in order that the care offered is family centred, sensitive and responsive to service users.
Clinical & Professional Leadership
- To provide clinical and professional Nursing leadership across the hospital site.
- To act as an advisor to the Director of Nursing on matters relating to professional conduct or Nursing and Midwifery Practice issues that may arise within the service.
- Be responsible for keeping his/her own practice up to date and maintaining competency relevant to the role. Also keeping up to date with national service requirements.
- To develop and maintain excellent communication throughout the service and organisation and others involved directly or indirectly with patient care to ensure a patient focused service.
- Link with senior medical staff when required regarding the management of patients.
- To support nursing staff within the service with clinical decision-making thereby enhancing their professional development.
- To serve on working groups or committees deemed appropriate to the role or grade.
Performance and Financial Management
- To assist the Director of Nursing in implementing any corrective programme necessary for adherence to financial budgets.
- To have a full awareness and understanding of the suite of performance indicators for the service which support the achievement of the hospital-wide Key Performance Indicators
- Establish mechanisms which provide the Director of Nursing with timely management information regarding performance against specified targets and budgets.
- To promote a corporate approach to the hospitals overall strategy objectives, mission and values.
Quality Assurance and Risk Management
- To ensure there is an effective process for risk assessment and risk management in respect of the service.
- To be responsible for ensuring that adverse incidents are investigated promptly, respecting confidentiality taking preventative action where necessary to ensure that IQS procedures are adhered to and fully implemented.
- To investigate patients' complaints and report appropriately in accordance with the hospital protocol.
- To participate where appropriate in formulation of policies, clinical guidelines, operational policy/procedures, sharing of information.
- To ensure that the hospital's health and safety, fire and other statutory regulations are adhered to
- To liaise with the Risk Manager and Quality Manager as appropriate.
- Co-operates and participates in quality initiatives against which clinical practice can be measured.
- Participates in audits of nursing practices and procedures in operation.
- Assists in identifying specific areas in which improvements can be made in effectiveness of care, and works to achieve these improved standards.
- Reports problems, which interfere with clinical standards of care, to the Director of Nursing.
- To work with the Director of Nursing and the senior nursing team on the development of the nursing strategy and ensure its implementation
- To work with Nursing and Multidisciplinary teams across the hospital in relation to review/development of processes, policies and procedures relative to JCI accreditation.
HR Management and Organisational Development
- Provision of direct line management to support staff within the service. Ensuring a period of Induction, scheduled probation meetings, and opportunity for personal growth and development.
- Ensure that the responsibilities and objectives for direct reports are clearly defined and understood – job descriptions and person specifications up-to- date.
- Be responsible for managing sickness and absence in accordance with hospital policy.
- Promote equal opportunity and adherence to best practice employment and approaches to managing and implementing change.
- To monitor attendance and allocation of all agency nurses and Health care assistants.
- In collaboration with other disciplines, promote good industrial relations in accordance with personnel policies.
- Involvement in the selection and recruitment of staff.
- Involvement in the planning and delivery of orientation/induction of relevant staff and participate in in-service training.

Candidates must be:
- Registered, or be eligible for registration in the General Nurse Division of the Register of Nurses and Midwives, maintained by the Nursing & Midwifery Board of Ireland
- Have 10 years post registration nursing experience and 5 years nursing management experience at a minimum of CNM2 in an acute setting
- Possess a post graduate qualification at not less than level 8 (QQI) in health care management related area
- Candidates must possess the requisite clinical, leadership, managerial and administrative knowledge and ability for the proper discharge of the office
