We are seeking a dynamic and compassionate Registered Nurse to fill a Client Care Manager position and to join our healthcare team. Responsibilities
- Act as the primary point of contact for the organisation patients and their families.
- Coordinate with multidisciplinary teams including nurses, social workers, physicians, and case managers to develop personalized care plans.

- Conduct intake assessments and care plan set up .
- Perform utilization review and utilization management to ensure appropriate resource use aligned with clinical guidelines .
- Manage case activities including discharge planning, follow-up care arrangements.
- Ensure compliance with HSE & HIQA regulations by safeguarding patient information during all interactions and documentation processes and audit ready .
- Proven experience in critical care environments.
- Strong background in hospital settings /community with, , or acute care nursing.
- Experience working within hospice care or behavioral health programs is a plus.
- Familiarity with social work principles, discharge planning procedures, and managed care processes..
- Excellent communication skills combined with the ability to handle intake assessments efficiently and empathetically.
- Ability to navigate complex healthcare regulations including HIPAA compliance standards effectively.
- Communication & Stakeholder Management*Coordinate and manage care schedules for clients and carers.
*Maintain effective regular communication with carers, clients and families regarding scheduling.
*Handle new client enquiries and HSE referrals.
*Support recruitment and onboarding of new care staff
*Provide administration support to the care team and compile weekly reports (Word, Excel)
*Ensure consistent application of companies policies, procedures and approved practice and to promote the aims of Right at Home.
*The candidate should have flexibility of being on call and travel within CHO3
