We are seeking an energetic and enthusiastic person to become part of our ever-expanding team in providing direct support on a 1-2-1 basis to adults on the Autism Spectrum. (inc. Mental Health challenges) within our Donegal town service location.

What We're Looking For

- Have a relevant third level qualification (min Degree Level) in a Health Studies discipline (Health & Social Care) or Disability Sector. Experience working with people with ASD is desirable.
- You must also hold a current full clean driving licence and have suitable means of transport available.

Job Description
Key Responsibilities
- Provide individual and/or group support based on assessed needs (social, educational, employment, independent living, emotional wellbeing, etc.)
- Deliver supports in line with person-centred principles and through bespoke individual plans
- Implement supports in accordance with our Making Connections / Connect 4 Life programmes and the HSE New Directions policy
- Monitor, review, and record members’ progress in line with our programme guidelines
- Ensure the health, safety, and welfare of members at all times
- Source, organise, and facilitate access to training, services, social activities, and employment supports
- Deliver outreach supports at off-site locations as required
- Build and maintain effective communication networks with families, employers, community organisations, and relevant services
- Liaise regularly with the APP Manager, Programme Staff, and the required external professional teams
- Attend review meetings, staff meetings, and supervision sessions
