Healthcare Coordinator (CNM 1) full time - Maternity Leave Specific Purpose Contract

Dara Residential Services (Dara) is a Section 39 organisation based in Celbridge, Co. Kildare. Dara provides residential, respite and individualised home and day support services to adults with intellectual disabilities. The Health Care Co-ordinator is responsible for the clinical oversight, coordination and continuous improvement of healthcare services across the organisation.
The Healthcare Coordinator will ensure that the people we support have timely access to high-quality, person-centred healthcare in the community. The role promotes health, wellbeing, rights, supported decision-making, and positive health outcomes. It involves working closely with Dara staff teams and community healthcare professionals. The role is based on the social model of disability. Services are mainly provided in Celbridge, with some support delivered in North County Dublin.
Essential Requirements

Be a Registered Nurse with current registration on the Nursing and Midwifery Board of Ireland (NMBI) in one of the following divisions: Intellectual Disability, General or Mental Health.
- Minimum of three years post-registration experience supporting adults with intellectual disabilities in a community, residential or healthcare setting.
- Demonstrate a strong commitment to rights-based practice, person-centred care, supported decision-making and capacity building in line with the Assisted Decision-Making (Capacity) Act 2015.
- Possess excellent knowledge of HIQA Standards, Health Act 2007, HSE policies, safeguarding, medication management and other relevant legislation and national standards.
- Have experience coordinating healthcare services and working collaboratively with General Practitioners, Primary Care professionals and other multidisciplinary professionals.
- Demonstrate experience in illness management, healthcare planning and supporting people with changing healthcare needs.
- Have experience developing, implementing and reviewing person-centred healthcare plans, including epilepsy, diabetes, nutrition and weight management plans.

Have experience delivering education, coaching and competency-based training to frontline staff and promoting health education for persons supported.
- Demonstrate experience in medication management, including Safe Administration of Medication, medication governance, auditing and quality improvement.
- Possess excellent leadership, organisational, communication, report writing, presentation and IT skills.
- Be capable of working independently while providing clinical leadership, guidance and support across multiple community locations.
- Demonstrate a flexible and proactive approach, responding effectively to the changing healthcare needs of persons supported.
Hold a full, clean Category B driving licence and have access to a vehicle suitable for the requirements of the role.

Desirable Criteria

Experience in a CNM1, Clinical Nurse Specialist or healthcare leadership role.
- Experience supporting nursing students as a preceptor or clinical supervisor.
- Experience undertaking clinical audits, quality improvement initiatives and healthcare governance.
- Qualifications in ageing in intellectual disability, dementia, palliative care and positive behaviour support

Experience developing healthcare policies, procedures and clinical guidance.
- Qualification in epilepsy management, diabetes care, dementia care or another relevant clinical speciality.
- Experience of preparing for and participating in HIQA inspections.
Terms
37.5 hours full time role - Maternity Cover Specific Purpose Contract.
Hours are mainly Monday to Friday 9:00 a.m. - 5:00 p.m or 8.30 a.m. - 4.30 p.m.
Salary is based on CNM 1 February 26 HSE consolidated payscales.
Point of scale will commensurate with relevant experience.
