The Role:
The Care Manager will play a pivotal leadership role in ensuring the delivery of high-quality, safe and person-centred care while supporting the continued growth and development of Heritage Homecare.
Key responsibilities will include:

Overseeing the consistent delivery of care to ensure the highest standards of quality, safety and regulatory compliance.

Working closely with the Quality Team to maintain excellence in service delivery, compliance and operational standards.

Building and maintaining positive, collaborative relationships with clients, families, healthcare professionals and other key stakeholders.

Supporting clients and families throughout their care journey, ensuring they feel informed, reassured and confident in the service they receive.

Liaising with the HSE regarding new client referrals, coordinating new packages of care and ensuring appropriate resources are in place.

Working closely with the Operations and Scheduling teams to optimise existing care resources and ensure continuity of care.

Supporting business development and service growth, identifying opportunities to expand our services and strengthen relationships with referral partners and other stakeholders.

Working towards agreed monthly and quarterly targets for new referrals, client growth and care hours.

Representing and promoting Heritage Homecare professionally within the community and helping to build awareness of our services.

Essential Requirements:

A nursing or clinical background is essential
-
Excellent verbal and written communication skills, with strong attention to detail

Strong leadership and organisational skills, with the ability to manage multiple priorities and work effectively under pressure

Ability to use initiative, work independently and collaborate effectively as part of a team
-
Strong interpersonal skills and the ability to build positive relationships with clients, families, colleagues and healthcare professionals

Full, clean driving licence and access to a car
Desirable experience:
-
Previous management experience an advantage but not essential. We welcome applications from nurses looking to move into their first management role.

Previous experience in Homecare/Community based care an advantage but not necessary.
