Job Scope:
The role of the Dedicated Neurology Nurse Manager involves supporting the existing patient caseload alongside the onboarding and management of patients whilst providing high quality clinical care. The Dedicated Manager will also be responsible for the management of their team members and their patient caseloads.
The Dedicated Neurology Nurse Manager will be required to fulfil all responsibilities for all scheduling, co-ordinating, facilitating and providing clinical care to the entire patient support programme in conjunction with the neurology team.
This is a permanent, full-time, Monday-to-Friday, nationwide role, primarily operating across Leinster and Munster.
Duties and Responsibilities:
Service Delivery:
The management of this Dedicated Nursing Service will include the following elements:
1. Supporting the existing patient caseload alongside the onboarding and management of new patients whilst providing high quality clinical care.
2. Responsible for all scheduling, co-ordinating, facilitating and providing clinical care to the entire patient support programme in conjunction with the with the neurology team
3. Assessment in the referring unit prior to initiating on treatment if requested by the referring unit.
4. Working with referring units to facilitate the transition of Patients from Hospital to the Home Support Service.
5. Attendance in the Hospital/Home to support the initial training
6. Post hospital home support/training Visit
7. On-going telephone support at pre-approved time intervals
8. Maintenance Visits with the Patient at Home at pre-approved time intervals
9. Provision of Reactive Visits
10. Staff training in hospital/respite facilities
11. Key contact for all communication from the multi-disciplinary teams
12. Liaising with the referring units regarding individual Patient progress and feedback
13. Key contact for the patient/family members
14. Key contact for team members and internal stakeholders
15. Provide clinical review, oversight, assessment and advise to our nursing team supporting the designated services.
16. Assist in ensuring that continuing quality systems are implemented and maintained within the TCP Group.
17. Provision of overnight services/long distance driving where required as role is nationwide coverage.
18. To provide support as part of the nurse team during the working hours
19. To respond and resolve any problems resulting from a helpline call
20. To feedback any helpline calls to nurse team colleagues
21. To document helpline calls in patient's notes
22. Represent self in a professional and ethical manner at all times, complying with all applicable company policy and procedures while adhering to the IPHA code of practice and Nursing and Midwifery Board code of conduct.
23. To demonstrate a commitment to the identification of risk, reporting all incidents and near misses.
24. Recognise and report immediately any untoward events on a patient visit. Report all adverse events and product complaints promptly in line with Pharmacovigilance procedures.
25. To adhere to and follow all policies and procedures in regard to decontamination of equipment and principles of infection control.
26. To facilitate and actively participate in liaison meetings between hospital referral units, the programme sponsor and TCP to maximise the value of the service and share key learnings.
Additional Responsibilities:

To manage the existing full patient caseload of the Neurology service and co-ordinate, lead and schedule all new patients referred to the service.

To closely co-ordinate, streamline and manage the service maintaining at all times the highest clinical standards.

To review referrals, service processes and patient safety initiatives.

To be the main point of contact to all stakeholders internal and external and to provide all relevant support and education for the multidisciplinary teams within the units.

To identify and deliver any proposed service enhancements as agreed from time to time with the client.

To meet the service Key Performance Indicators as agreed with the client.

Education of clients/families and colleagues in relation to relevant disease areas.

Timely electronic reporting at each visit.

Maintenance of accurate and detailed records as agreed for each patient.

To liaise closely with all other healthcare personnel associated with services ensuring good lines of communication with the hospitals, consultants and other clinical personnel.

To liaise closely with the patient, if the patient requires additional visits

Assess specific changes in the patient's condition as a result of new or on-going treatments and take immediate appropriate action.

Maintain customer confidentiality at all times.

To prepare patient schedules/ nurse visits in a timely manner

Implement and update all TCP Homecare operating procedures and protocols relating to the care and safety of a patient and yourself.

Attend relevant meetings with company and/or clients to provide oversight and details of the service and facilitate meticulous feedback.

Participate in approved data collection, clinical studies and trials as requested by the client.

Keep abreast of current trends and developments in professional matters and in clinical care. Ensure that clinical skills are maintained.

Responsible for the administration of daily/weekly/monthly documentation for departmental reviews.
Requirements:
Qualified RGN- NMBI registered
Neurology background highly desirable
Managerial experience highly desirable
Must possess a strong work ethic and ability to work with team members while demonstrating ability to work under own initiative
Must have the ability to educate, communicate and present while building a rapport with a range of key stakeholders
Must have a systematic and structured approach to managing client services
Must be flexible, adaptable and available to travel nationwide with some overnight stays required
Must have a full clean driving licence
Must be competent in Microsoft excel, outlook & PowerPoint
