Responsibilities:
Provide person-centred care and support to individuals in accordance with care plans.
Foster a positive and supportive environment for clients, promoting independence and well-being.
Collaborate with multidisciplinary teams to ensure comprehensive care delivery.
Maintain accurate and up-to-date records of client interactions and progress.
39hr/week
Requirements:
Relevant qualification in social care.
Proven experience as a Social Care Worker and leadership experience .
Strong interpersonal and communication skills.
Ability to work independently and as part of a team.
Empathy, compassion, and a commitment to delivering high-quality care.
