emeis Ireland are currently recruiting for a Social Care Facilitator who will provide meaningful activities and stimulation to all residents at our nursing home in Maynooth Lodge. The post involves working as a part of a team, assessing residents’ individual interests and acknowledging individual differences between residents. Social Care Facilitators will encourage and promote participation in appropriate activities that allow for an increased sense of wellness and enjoyment in life.
Key Responsibilities:
- Assessing residents' psychosocial needs and helping develop person-centred care plans.
- Organising and leading group and individual activities that support residents' social, emotional, and developmental well-being.
- Advocating for residents' rights and supporting their independence and dignity at all times.
- Facilitating communication between residents, families, and staff, ensuring sensitive and timely information sharing.
- Providing emotional, practical, and bereavement support to residents and their families as needed.
- Monitoring, recording, and reporting on residents' progress and participation in activities.
- Supporting and encouraging residents to participate in community and recreational events.
- Maintaining accurate and confidential records as per organisational policy.
- Contributing to the maintenance of a safe, positive, and respectful environment for all residents.
- Making appropriate referrals to additional services and integrating families into support resources as required.
