Purpose
The successful applicant will be a valued member of our Stewarts Day Service Team, supporting the health and wellbeing of service users to achieve person-centered goals and ensuring their clinical practice is underpinned by NMBI requirements and Stewarts Care Policies, Procedures, Protocols and Guidelines.
Key Duties and Responsibilities
Professional Responsibilities
The post holder will:
Provide oversight and governance of the assigned day service.
Promote the health, welfare and social wellbeing of service users.
Manage day to day planning and activities of the assigned day services.
Take the lead role in implementing day services programmes as per New Directions for service users.
Manage assigned caseload of service users.
Develop and complete health assessments and associated care plans to address existing and emerging health needs for service users.
Delegate to and supervise the work of other grades of staff within the remit of their role, as appropriate.
Healthcare and Social Wellbeing Supports
Support the service user to develop person centred plans.
Assess, plan, implement and evaluate individual person-centred programmes within New Directions framework and in accordance with best practice.
Provide medication management supports for service users in line with Stewarts Care policies.
Develop and promote good interpersonal relationships with service users, their family / circles of support and the interdisciplinary care team in the promotion of person-centred support.
Support RSAMM trained staff competency assessments and medication audits.
Provide information and education for service users and support staff on developing healthcare plans.
Regular review with the service users and their families/circles of support where relevant of their health and social wellbeing needs.
Monitor and review of health-related recording forms.
Develop and review the HSE healthcare passport to reflect current needs and supports.
Support service users with person centred needs in identifying requirements for attending screening programmes and diagnostics e.g. National screening programmes, phlebotomy, bone density etc.
Communicate effectively with the service user, family/circles of support and acute medical nursing team when inpatient for planned or emergency hospital admission to ensure effective communication, recording of progress/presentation and support safe discharge planning back to day service.
Communicate with service users and their family/circles of support (where relevant) during periods of illness causing absence from day services to provide supports for return.
Communication and Multi-disciplinary Supports
Engage with Hub leads in regard to service users’ healthcare needs and in particular declining/evolving needs and supports.
Support service users with assisted decision making by optimising access to information and applying reasonable accommodations.
Engage with outreach and community services in support of individual service users, where required.
Attend and contribute at MDT and annual review for service users for service users in regard to health & social wellbeing needs.
Promote and recognise the service users’ social and cultural dimensions of care and the need for links with their local community.
Provide competency review and supports to non-nursing staff in the area of medication management and enteral feeding.
Accurately record and report all complaints to their line manager.
Nursing Practice
Practice Nursing according to the Code of Professional Conduct as laid down by the Bord Altranais agus Cnáimhseachais na hÉireann (Nursing Midwifery Board Ireland) and Professional Clinical Guidelines
Adhere to national, regional and local Health Service Executive (HSE) guidelines, policies, protocols and legislation.
Record nursing practice on eclipse system in a timely manner in accordance with NMBI requirements.
Report any safeguarding concerns for service users as per the StewraTS Safeguarding policy to their line manager.
Ensure completion of required records and reports such as SIMS.
Support the development and review of SOP’s required for nursing practice.
Deliver and report on KPI’s and updates to the line manager for compliance tracking.
Engaging in team meetings with line manager in relation to team and service planning.
Support audit and reports in regard to New Directions policy.
Provide preceptorship and mentorship for undergraduate students.
Ensure evidence-based practice in line with national and international best practice and policy e.g. National wound care guidelines,
Manage upkeep appropriate levels of equipment and supplies required in the role.
Engage in continuous professional development relevant to the role and engage in the national Professional Development Plan (PDP)
Administration
Ensure that records are safeguarded and managed as per HSE / local policy and in accordance with relevant legislation
Maintain records and submit activity data / furnish appropriate reports to the Director of Nursing as required
Contribute to policy development and formulation, performance monitoring, business planning and budgetary control.
Maintain professional standards including patient and data confidentiality.
Contribute to the development and implementation of information sharing protocols, audit systems, referral pathways, individual care plans and shared care arrangements.
Contribute to ongoing monitoring, audit and evaluation of the service as appropriate.
Ensure that the care setting is maintained in good order using appropriate models, that supplies are adequate, and that all equipment is in good working order and ready for immediate use.
Ensure that equipment is safe to use and report any malfunctions in a timely manner

Candidates must demonstrate sufficient competency and professional knowledge to carry out the duties and responsibilities of the role.
Education and Skills
Essential Criteria:
Excellent communication skills both verbal and non-verbal.
Minimum two years’ experience in ID.
The aptitude to provide a person-centred approach to service user programmes.
The ability to develop implement and monitor such individual programmes.
Organisational & Role Specific Knowledge
Demonstrates knowledge of the services provided by the organisation.
Knowledge of the organisational structure and functions.
Awareness of the organisation’s Strategic Plan
Awareness of national policies and legislation shaping the disability sector.
Communication
Effective communication skills both verbal and written.
Understanding the importance of the MDT in the provision of care and the ability to develop and maintain relationships with all stakeholders.
Ability to engage and negotiate with relevant stakeholders in the best interests of service users.
Proficient in Microsoft suite and strong ICT skills relevant to the role.
Ability to work on own initiative.

Qualifications & Experience

Candidates must meet the following criteria relevant to the role:
Currently registered with Nursing and Midwifery Board of Ireland in the RNID Division.
Applications from other nurses registered with Nursing and Midwifery Board of Ireland who have a minimum of 2 years’ experience in the area of Intellectual Disability will also be considered. Successful candidates from non RNID division will be required to complete post graduate education in the ID field.
Candidates will possess a high level of communication skills, work well within a team and be able to provide high quality focused care
Must have strong command of English language (both verbally and written) and be eligible to work in Ireland
Candidates should be proficient in I.T.
Desirable
Full driver’s licence and access to own car.

Intellectual Disability Community Nurse
Remuneration is in accordance with the Department of Health consolidated pay scales
Current Salary Scale: Staff Nurse, Intellectual Disabilities, €€38,166 - €56,592 incl LSI per annum.
Salaries and increments are paid in accordance with DOH salary scales

The hours allocated to this post are 37.5 hours per week exclusive of lunch breaks. The allocation of these hours will be at the discretion of the Director of Nursing and in accordance with the needs of the service.
All employees will be subject to a probationary period of 6 months, with an extension if required to 9 months. This probationary period applies to all employees irrespective of their role or type of role.
Stewarts Care will carry out Garda vetting on all new employees. An employee will not take up employment with the organisation until the Garda Vetting process has been completed and the organisation is satisfied that such an appointment does not pose a risk to the service users and employees.
