Shift Pattern:Monday - Sunday 36 Hours per week
Main duties and responsibilities
- Monitoring and reporting patients' health needs. and contributing to the development of care in care plans and complete interventions as described.

Supporting the administration of medications and treatments as prescribed. and providing emotional support to residents.

Collaborating with a multidisciplinary healthcare team to ensure comprehensive patient care.

Communicating effectively with physicians, nurses, support staff, and external allied health practitioners.

Participating in team meetings, handovers, and performance reviews. and maintaining accurate and up-to-date patient records.

Documenting all interventions and patient responses. and ensuring compliance with best practice in a private home care setting.

Responding to emergencies and providing immediate care as per policy. and collaborating with emergency response teams as needed.

Working in accordance with Health & Safety standards and using the provided personal protective equipment.

Demonstrating flexibility in response to business changes, development, and the review of best practices.

Helping clients with personal care tasks such as bathing, hair care, shaving, dressing, and undressing.

Helping with meal preparation, considering dietary preferences and restrictions.
-
Assisting at mealtime, including eating support if necessary. and gently reminding and supporting individuals to take prescribed medications.

Assisting with medication support and administration under the supervision of qualified healthcare professionals,
- Participating in recreational activities to promote emotional well-being. and providing emotional support and a caring presence.

Performing light housekeeping duties, including cleaning, laundry, and organising.

Maintaining open and clear communication with individuals, their families, and healthcare professionals.

Reporting any changes in the individual's condition or concerns to the appropriate supervisor.

Monitoring individuals for safety and aiding in preventing accidents.

Assisting with the use of mobility aids, such as walking of wheelchairs.

Staying informed about the specific needs and conditions of the individuals under care.

Ensuring the client's dignity and independence and honouring the preferences and choices of individuals under care.
Qualifications and experience
- Minimum QQL 5 qualifications

Excellent communication skills and customer service
