At Comfort Keepers Ireland, we are seeking a full time permenent RGN Case Manager to lead and coordinate a person-centred care package for an individual living with an Acquired Brain Injury (ABI) in their own home in Enfield, Co. Meath.
Combining clinical nursing, case management and leadership, you'll work closely with the client, family, care team and multidisciplinary professionals to support their health, rehabilitation, independence and quality of life.

Lead the assessment, planning and ongoing review of the client's care.

Monitor clinical needs and respond appropriately to changes.

Coordinate care with GPs, hospitals and multidisciplinary professionals.

Support medication management and complex healthcare needs.

Provide clinical guidance and support to the care team.

Promote rehabilitation, independence and personal goals.

Manage clinical risk, safeguarding and quality standards.

Ensure accurate and up-to-date clinical documentation.

Registered General Nurse (RGN) with current professional registration.

Minimum 3 years' post-registration nursing experience.

Experience in case management, community nursing, rehabilitation, disability and/or complex care.

Strong clinical assessment and care-planning skills.

Confidence working autonomously and making clinical decisions.

Strong communication, leadership and organisational skills.

Commitment to person-centred, evidence-based care.

Experience with ABI, neurological conditions or complex care is highly desirable.
