Occupational Therapist – Elder care
Medforce are seeking an Occupational Therapist to join our clients team supporting older adults across assessment, rehabilitation, and safe discharge planning. You will provide holistic, person-centred interventions that promote independence and functional wellbeing. Working as part of a dynamic multidisciplinary team, you will lead on complex cases and contribute to high-quality patient flow. This role is based close to Dublin city centre on all transport links and accessible by car.
Requirements of Occupational Therapist

Significant post-qualification experience working with older adults in acute, community, or intermediate care settings.

Demonstrated ability to complete holistic and functional assessments with older adults, including those experiencing frailty, cognitive impairment, or multi-morbidity.

Experience delivering patient-centred rehabilitation programmes aimed at maximising independence and reducing hospital length of stay.

Proven competence in managing complex discharge planning, including coordination with multidisciplinary teams and external agencies.

Ability to assess for, recommend, and provide assistive equipment and minor adaptations to support safety and independence at home.
- Strong understanding of age-related conditions, functional decline, falls risk, and dementia-related needs.

In-depth knowledge of local and national policies related to discharge to assess (D2A), hospital flow, and community support pathways.

Clear understanding of safeguarding principles for adults and how to escalate concerns appropriately.

Working knowledge of compensatory strategies, graded activity, risk assessment, and manual handling principles.
- Excellent clinical reasoning and problem-solving skills.

Ability to plan, deliver, and evaluate rehabilitation interventions tailored to older adults’ goals.

Strong communication skills to work effectively with patients, carers, MDT colleagues, and social care partners.

Ability to write clear, concise clinical documentation and reports.

Confident in providing education to patients, families, and staff regarding safe discharge and home management strategies.

Capacity to prioritise workload and manage time effectively in fast-paced settings.

Previous experience in integrated care systems, rapid response, or reablement services would be beneficial
