Patient Flow Coordinator (CNM1)

We are currently recruiting for full-time Patient Flow Coordinator for our Beaumont Lodge Transitional Car Unit, in Artane, Dublin 5.

Purpose of The Role:

To support the multidisciplinary team by scheduling, daily admissions, and coordination of bed management at Beaumont Lodge Transitional Care Unit, and to discharge bed management function in accordance with Bartra Healthcare Standards of Quality Care.

Roles and Responsibilities:

To Develop rapport with critical stakeholders.
To Ensure that referral, admissions and discharge process is adhered to at all times.
To Conduct a thorough clinical assessment of each patient referrals either on site or via telephonic assessment as appropriate.
To Allocate patient to the most appropriate ward/bed available to assure safe standard of nursing care.
To Monitor bed occupancy and send daily reports to senior management.
The person holding this post is required to support the principle that the care of the patient always comes first and will approach their work with the flexibility and enthusiasm necessary to make this principle a reality for every patient to the greatest possible degree.
Ensure that the provision of a high standard of care to the patient or patients is consistent with the mission, vision, values and strategic plan of Bartra Healthcare
Be responsible for the following in relation to Governance, Leadership and Management:
In the absence of the Assistant Director of Nursing, undertake the role and responsibilities of the Person-in-Charge in line with the Health Act 2007, related regulations and the Health Information and Quality Authority (HIQA) standards.
Support the Registered Provider/Director of Nursing the day to day management of the Place of Work.

Qualifications

Are registered in the relevant division of the Register of Nurses & Midwives maintained by the Nursing and Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.
Evidence of continuous professional development.

Experience

Essential:

Have at least 5 years post registration experience (or an aggregate of 5 years fulltime post registration experience) of which 2 years must be in the speciality or related area. A minimum of two years nursing experience in the related area.
Have the clinical, managerial and administrative capacity to properly discharge the functions of the role.
Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical, managerial and administrative capacity to properly discharge the functions of the role.
