The candidate must have 5 years post-graduation experience (or an aggregate of 5 years full time)
- A minimum of 2 years community nursing experience is required and preferably has had previous working experience with the Traveller community/ vulnerable communities.
- Must demonstrate an ability to case manage clients referred to them and work alongside the existing PHN service to manage health needs identified.
- The role requires an in- depth knowledge of services within primary care and the multidisciplinary team.
- May be required to work flexible hours if evening workshops were required.
- Candidate must have access to appropriate transport to fulfil the requirements of the role
