On behalf of our client in public health, we are seeking a motivated and compassionate Social Worker to join the multidisciplinary team within a Special Care setting, supporting young people with complex needs and high-risk behaviours. The successful candidate will work collaboratively with young people, families, residential staff, external agencies, and healthcare professionals to promote positive outcomes, safeguard welfare, and support care planning and intervention strategies.
- Undertake comprehensive social work assessments of young people referred to or placed within Special Care services.
- Contribute to the development, implementation, and review of individual care plans and therapeutic interventions.
- Work directly with young people and their families to support engagement, family relationships, and transition planning.
- Participate in admission, care planning, and discharge processes for young people within the service.
- Liaise closely with residential teams, healthcare professionals, educational services, community agencies, and statutory bodies to ensure coordinated service delivery.
- Prepare reports, assessments, and documentation to a high professional standard and maintain accurate case records.
- Attend and contribute to multidisciplinary meetings, care reviews, case conferences, and court proceedings where required.
- Provide advice and guidance to colleagues on social work matters, safeguarding concerns, and family support interventions.
- Promote best practice and ensure compliance with relevant legislation, policies, and professional standards relating to Special Care services.
- Support young people in achieving positive outcomes in relation to education, health, emotional wellbeing, and social inclusion.
- A recognised qualification in Social Work.
- Registration or eligibility for registration with the relevant professional regulatory body.
- Knowledge and understanding of child protection and welfare legislation, including relevant Irish legislation and national policy frameworks.
- Experience working with children and adolescents presenting with complex needs, trauma, behavioural challenges, or involvement with child protection services is desirable.
- Excellent communication, interpersonal, advocacy, and report-writing skills.
- Ability to work effectively both independently and as part of a multidisciplinary team within a challenging and dynamic environment.
- Demonstrated ability to build positive relationships with young people, families, and external stakeholders.
- A full clean driving licence and access to transport may be required.
