As a friendly, patient-focused GP practice committed to delivering high-quality, compassionate primary care we are seeking an experienced and motivated GP Practice Nurse to join our multidisciplinary team on a full-time, permanent basis.
The successful candidate will play an important role in providing clinical care, supporting the management of chronic conditions, promoting preventive health and ensuring patients receive a professional and positive experience.
Duties and Responsibilities
The GP Practice Nurse will be responsible for:
- Providing safe, evidence-based nursing care to patients of all ages.
- Conducting patient assessments, recording clinical observations and maintaining accurate healthcare records.
- Supporting the management and monitoring of chronic conditions, including diabetes, asthma, COPD, hypertension and cardiovascular disease.
- Conducting chronic disease management reviews in line with relevant clinical guidelines and practice protocols.
- Administering childhood, adult, travel and seasonal vaccinations, where appropriately trained and authorised.
- Performing cervical screening and supporting other national screening programmes, where qualified.
- Carrying out phlebotomy, blood pressure monitoring, ECGs, 24hr ABPMs, urinalysis, and other routine clinical procedures.
- Providing wound assessment, wound care, dressing changes and suture or staple removal.
- Assisting with minor surgical procedures and maintaining appropriate infection-prevention standards.
- Providing health promotion, lifestyle and preventive-care advice, including smoking cessation, weight management and cardiovascular risk reduction.
- Supporting women’s health services including contraception reviews
- Providing patient education on medication, treatment plans, self-management and follow-up care.
- Identifying and responding appropriately to deteriorating patients or urgent clinical concerns.
- Managing clinical stock, vaccines, emergency equipment and cold-chain records.
- Ensuring clinical rooms and equipment are clean, organised, properly maintained and ready for use.
- Following infection-prevention and control procedures and contributing to relevant audits.
- Maintaining complete, accurate and confidential patient records using the practice’s clinical software.
- Communicating effectively with GPs, nurses, administrative staff, community services, hospitals and other healthcare professionals.
- Participating in clinical meetings, quality-improvement initiatives, audits and practice development.
- Maintaining professional competence through continuing education and training.
- Working in accordance with professional standards, clinical policies, data-protection requirements and the nurse’s defined scope of practice.
- Undertaking other appropriate nursing duties required to support the effective operation of the practice.
Essential Requirements
- Current registration with the relevant nursing regulatory body.
- Previous nursing experience in general practice, primary care, community care or a similar clinical environment.
- Strong clinical assessment and patient-care skills.
- Competence in phlebotomy, wound care, vaccination and routine clinical procedures.
- Excellent communication, organisational and interpersonal skills.
- Ability to work independently while contributing positively to a multidisciplinary team.
- A professional, compassionate and patient-centred approach.
- Strong attention to detail and a commitment to maintaining patient confidentiality.
Desirable Requirements
- Experience in chronic disease management.
- Training or experience in cervical screening.
- Vaccination and immunisation qualifications.
- Experience in women’s health, travel health or health promotion.
- Experience using GP practice management or electronic patient-record systems.
- Certification in basic life support, anaphylaxis management and infection prevention and control.
