Registered Nurse – Community Care Manager (NMBI Registered)

Location: CHO3 Region

Job Type: Permanent, Full-Time (Monday to Friday)

Salary: Competitive, depending on experience

About the Role

We are seeking an experienced, compassionate and motivated NMBI Registered Nurse to join our team as a Community Care Manager. This is an exciting opportunity for a nurse who is passionate about delivering high-quality, person-centred care while leading and developing community care services.

As Community Care Manager, you will be responsible for the clinical leadership, operational management and day-to-day coordination of our home care services. You will work closely with clients, families, carers, the HSE and multidisciplinary teams to ensure the highest standards of care, compliance and service delivery.

This is a varied role that combines clinical expertise, leadership, quality assurance, workforce management and community engagement.

Key Responsibilities

Clinical Leadership & Care Management

* Provide clinical leadership and oversight of community care services.

* Conduct comprehensive client assessments and establish individualised care plans.

* Review and update care plans to ensure they meet clients’ changing needs.

* Monitor clinical outcomes and ensure delivery of safe, effective and person-centred care.

* Provide clinical guidance and support to care staff.

* Coordinate multidisciplinary care with GPs, public health nurses, hospital teams, social workers and allied healthcare professionals.

* Support discharge planning and transitions from hospital to home.

* Ensure appropriate utilisation of resources while maintaining excellent standards of care.

Daily Operations Management

* Oversee the daily operation of community care services.

* Coordinate and manage care schedules for clients and carers.

* Ensure continuity of care and appropriate staff allocation.

* Respond to operational issues and provide effective solutions.

* Participate in the on-call rota and provide out-of-hours support when required.

* Travel throughout the CHO3 region as required.

Department Performance

* Monitor service performance and key operational targets.

* Produce weekly and monthly operational reports.

* Identify opportunities for service improvement.

* Support business development and sustainable service growth.

* Ensure high levels of client satisfaction and quality outcomes.

Staff Management, Supervision & Development

* Recruit, interview and onboard new healthcare staff.

* Conduct staff inductions, supervision and performance reviews.

* Provide mentoring, coaching and ongoing clinical support.

* Identify learning and development opportunities for staff.

* Promote a positive, collaborative and professional working culture.

Recruitment & Workforce Planning

* Support workforce planning to ensure adequate staffing levels.

* Manage recruitment processes for healthcare assistants and nursing staff.

* Assist with staff retention initiatives.

* Ensure all staff maintain mandatory training and compliance requirements.

Clinical Governance, Compliance & Quality Assurance

* Ensure compliance with HSE, HIQA and all relevant regulatory standards.

* Maintain audit-ready documentation and clinical records.

* Lead quality improvement initiatives.

* Monitor incident reporting and risk management processes.

* Ensure policies, procedures and best practice guidelines are consistently implemented.

* Maintain confidentiality and safeguard client information at all times.

Communication & Stakeholder Management

* Act as the primary point of contact for clients and their families.

* Build strong relationships with the HSE, community healthcare professionals and external stakeholders.

* Manage new client enquiries and HSE referrals.

* Maintain regular communication with clients, families and carers regarding care arrangements.

* Represent the organisation professionally within the local community and healthcare sector

Care Plan & Assesment

*Conduct care pan and asseasment

*HIQA & HSE Audit ready operation

Administration

* Maintain accurate client records and documentation.

* Prepare reports using Microsoft Office applications.

* Support administrative functions within the care team.

* Ensure compliance with organisational policies and procedures.

Essential Requirements

* Current NMBI Registration.

* Full clean Irish driving licence.

* Access to your own vehicle.

* Right to work in Ireland.

* Minimum 1-3 years’ post-registration nursing experience.

* Previous experience in community nursing, acute hospital, clinical management or care management.

* Strong knowledge of HSE and HIQA standards.

* Excellent clinical assessment and care planning skills.

* Strong organisational and time management skills.

* Excellent communication and interpersonal skills.

* Ability to work independently and as part of a multidisciplinary team.

* Proficient in Microsoft Office. Knowledge in using IT tools

Desirable Experience

* Previous leadership or management experience.

* Experience in home care or community healthcare services.

* Experience with staff supervision and recruitment.

* Knowledge of quality assurance, governance and audit processes.

* Experience managing multidisciplinary teams.

* Experience in critical care, acute care, hospice or behavioural health is advantageous.
