Team Liaison Nurse, CNM2 Grade

St Patrick’s Mental Health Services seeks a Team Liaison Nurse to work as a member of the Nursing team in St Patrick’s Mental Health Services to ensure a high-quality service.
The Team Liaison Nurse will report to the Director of Nursing, professionally; Consultant Psychiatrist, Clinically and Programme Manager for R&A Service, Operationally.
The standard working week applying to the post is 37.5 hours per week, working Monday – Friday, with on average, one evening shift per fortnight.

Location
St Patrick’s Mental Health Services (SPMHS), incorporating St Patrick’s University Hospital, James’s Street, Dublin 8; St Patrick’s Hospital, Lucan; Willow Grove Adolescent Unit, James’s Street, Dublin 8; and the Dean Clinics.
Responsibilities
Main duties and responsibilities:
- For the purposes of completing an admission liaise with medical colleagues (e.g., Registrar in Admissions or Registrar on call) to conduct clinical formulation, ICD diagnosis, physical examination, and decision to admit as per MHC Code of Practice on Admission, Transfer and Discharge to and from an approved centre.

Complete admissions as per hospital policy and procedure using the Admission Psychiatric Assessment and Nursing Admission Assessment documents as required.
-
Engage with Service User’s family and/ or advocate with relevant information and advice regarding the admission process (including their rights under the Mental Health Act 2001) and care and treatment process while in hospital.
-
Make clinical decisions regarding the appropriate placement of service users following assessment for admission in conjunction with relevant clinical, medical, administrative and management staff.
-
Plan, coordinate and manage with CNM2/Bed Manager in Referral & Assessment Service to triage referrals for elective appointments for admission assessment.
-
Engage with Service Users to arrange times for admissions and assessments.
-
To work as a multidisciplinary team (MDTs) under the clinical lead of a designated Consultant as part of the MDT.
- Implement an evidence-based and recovery focused approach to practice in collaboration with service users, families, carers, and relevant external agencies, e.g., primary care, continuing-care etc.

Conduct comprehensive assessments including but not limited to:
-
Admission psychiatric history assessment including collateral history
- Mental State Examination
-
Fitness for therapeutic leave
-
Suitability for appropriate ward placement
- To inform decision to prescribe medication or adjust current prescriptions

To inform discharge planning
-
To inform longitudinal assessment of suicide risk
-
To inform the ARAT
-
Other assessments as required

Request blood tests and take blood samples as required.

Incorporate the service user’s physical, psychological, social circumstances, values and beliefs and heath history in all assessments and be considerate of their personal wishes.

Maintain and fulfil key working responsibilities as an MDT member.

To maintain service user, staff, and Hospital confidentiality in accordance with Hospital Policy.
- To practice with adherence to Nursing and Midwifery Board of Ireland’s Code of Professional conduct and Scope of Practice.

Conducting investigations and taking initiative to monitor treatments (such as lithium) commenced in hospital.
-
Initiating referrals to and liaising with other MDT members and programme coordinators.
-
Monitoring progress of service users on leave and communicating rationale and details of treatment plan including discharge arrangements.

Liaising with Dean Clinics and participating in pre-arranged outpatient clinics.

To engage in the provision of advanced practice roles commensurate with ones individually assessed competency as outlined in Nursing and Midwifery Board of Ireland’s, Scope of Practice document (e.g., IV fluid input, female catheterisation, wound care, and prescribing medication).

To employ audit systems to evaluate systems, processes, and outcomes for Mental Health services.
- To work with the assigned ADON to maintain links with service users transferred to external acute medical care e.g., in St James’s Hospital
- Provide a coordinating and leadership role regarding MDT recovery focused care and discharge planning.

TERMS AND CONDITIONS OF EMPLOYMENT
Team Liaison Nurse, CNM2 Grade

Tenure: 1.0 WTE Full-time, Specified Purpose contract, pensionable as below.
Working Week: The standard working hours applying to this post is 37.5 hours worked Monday to Friday for full time and working on average one evening shift per fortnight.
Salary: The Salary for the position is according to the CNM2 SPMHS 2026 salary scale.
Annual Leave The annual leave associated with the position is as appropriate to number of years post- qualification experience and appropriate to CNM2 grade.
Pension: All pensionable staff become members of the pension scheme upon commencement of employment. The scheme is a defined contribution scheme with a deduction from the salary of the employee of 4%.
In respect of an internal appointment being made the individuals existing pension arrangements will continue.
Probation: The appointee will be appointed in a probationary capacity for the first six months during which time the job holder’s performance will be subject to ongoing review.
Any extension to or suspension of the period of probation will be in line with the European Union (Transparent and Predictable Working Conditions) 2022.
Health: The appointee must be fully competent and capable of undertaking the duties attached to the position and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.
Character: The appointee must be of good character.
Retirement Age: Normal retirement age for employees is 66 years which at present is also normal pension age.
