Job Profile:
The role of Social Care Lead is to develop and lead an innovative and varied social and recreational program for residents living in Bridhaven Nursing Home in Mallow in order to improve residents' quality of living and social experience living in Bridhaven Nursing Home. The Social Care Lead is a key player within the multidisciplinary team, ensuring the necessary supports are put in place for each resident to maximise their independence for as long as is safely possible.
Key Objectives
Social

Lead the planning, implementation, and evaluation of person-centred activity programmes in Bridhaven for our Residents

To ensure the scheduling of social sessions provides maximum benefit to both the resident and the care team
- To encourage resident to join in meaningful activities whilst maintaining their independence, choice and dignity, to enable them to lead as full and active life as possible
- To consult with each resident in order to identify their social and recreational requirements according to their likes/dislikes, abilities and personal preference

To produce a monthly programme of activities and social events and ensure it is communicated to all residents.
- To regularly assess the effectiveness of the social events and activities programme.
- To work as a key member of the multidisciplinary team in order to feedback on each resident's status/participation at regular care plan reviews
- To establish and maintain relationships with residents that are based on respect and equality and that promote their independence.

Encourage resident participation while respecting individual choice and independence.
Administration

To familiarise and connect with all key community groups whose expertise would make a valuable contribution to the service.

To recruit a network of volunteers to provide a variety of desired appropriate social opportunities as identified To support volunteers in their role and ensure they receive appropriate induction, training, supervision and support.

Maintain accurate documentation and activity records in line with regulatory requirements.

Monitor budgets and manage activity resources effectively.

Ensure activities comply with HIQA standards, infection prevention procedures, health and safety regulations, and safeguarding policies.

Contribute to quality improvement initiatives and support inspections where required.

The successful candidate must hold the following qualifications and experience to fulfill the requirements of this role:
Essential
Degree in Social Care or relevant field
Previous experience working with the older person an advantage
